In order for a child to qualify for free treatment on the MAC Van, pertinent forms must be completed. No child will receive care with out these completed forms. Forms can be picked up at The Pettengill House at 13 Lafayette Road or the Boys and Girls Club of LMV at 18 Maple St., both in Salisbury.
